DE QUEEN, Ark.—Carolyn Ruth Hornsby Formby, 74, of De Queen died Saturday, April 13, 2019, in De Queen. Mrs. Formby was born Jan. 31, 1945, in Lambert, Miss. She was a homemaker and a member of Kern Heights Baptist Church. She was preceded in death by one daughter, Amy Beth Formby. Survivors include one son, Jason Formby; five brothers, Ronald Bert Hornsby, Arthur Grady Hornsby, Charles Everett Hornsby, Quinton Ray Hornsby and James Allen Hornsby; and a number of other relatives. Services will be 2 p.m. Thursday at Wilkerson Funeral Home with Bobby Fisher officiating. Burial will be in Redmen Cemetery. Visitation will be from 1 to 2 p.m. Thursday at the funeral home.
